Job Description:
Overview:
-Responsible for collecting, accessioning, processing, and
distribution of laboratory specimens both internally to the
hospital laboratory departments and externally to a referral
laboratory. Certified phlebotomists collect blood samples from both
inpatients and outpatients in order to assist in physician
diagnosis to provide quality patient care. -Talent Pool: Allied

Responsibilities:
- -Collects quality blood specimens, including maintenance of iSTAT
competency for quality POC testing if applicable. Expected
performance to include less than 2.5% blood culture contamination
rate, less than 1.0% patient recollections due to unsatisfactory
specimen integrity and no more than 1 patient complaint regarding
non-adherence or compliance of technical and/or behavioral
standards.
--------------------------------------------------Complies with all
regulatory standards. Demonstrates and adheres to hospital and
laboratory safety and compliance policies and procedures. Completes
annual competency by due date.
--------------------------------------------------Demonstrates
effective problem solving skills. Utilizes available resources for
complete resolution of suboptimal specimens. Assures optimum
outcome. Utilizes lab/hospital systems in order to accurately
document lab activity to include collection lists, pending logs,
un-received reports, and labels according to established site
protocols throughout the shift to ensure established turnaround
times are met. Expected performance is demonstrated by having less
than 1 delayed sample per month. Ensures appropriate response and
follow up on all customer service
issues--------------------------------------------------Ensures
accurate patient identification. Expected performance is
demonstrated by having zero rejected blood bank specimens, less
than 2 correctable blood bank labeling errors, zero misidentified
patient incidences and zero mislabeled
specimens.--------------------------------------------------Maintains
supplies (to include, but not limited to phlebotomy baskets, carts,
workspace and electronic and telecommunication devices). Expected
performance is demonstrated by having no more than 1 expired tube
found in carts or baskets, performs monthly cart checks as assigned
and ensures phlebotomy supply area is clear of expired products,
clean, organized and restocked daily. Electronic devices are to be
kept in appropriate condition and are to be turned over to next
shift in working
order.--------------------------------------------------Performs
all processes associated with specimens received into the
laboratory setting. Includes reviewing the test order
electronically or manually, ensuring patient labeling and positive
patient identification, accessioning specimens into the laboratory
information system, accessing specimen acceptability for testing
ordered, processing the specimen according to the test requirements
and distributing the specimen to the appropriate laboratory
department for
testing.--------------------------------------------------Prioritizes
assigned duties and workload to ensure completion meets established
laboratory turnaround

Qualifications:
-EDUCATION:High School Diploma or equivalent,
RequiredPreferred:Graduate from an accredited phlebotomy program.
Associates degree in a chemical, physical, or biological science
and/or one year of post high school education.
-EXPERIENCE:Preferred:A minimum of 6 months to 1 year, full time
experience in phlebotomy. (Preferably hospital
setting)LICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIREDBLS
(CPR)-American Red Cross or AHA Healthcare ProviderCurrent
phlebotomy licensure from one of the following approved agencies:
ASCP (American Society for Clinical Pathology), ASPT (American
Society of Phlebotomy Technicians), NHA (National Health career
Association)OR Current Licensed Advanced EMT/ParamedicPREFERRED
