About Us Our organization has been at the forefront of establishing
the first national in-home clinical network. Today, we stand as an
independent provider of thorough health assessments, catering to
individuals nationwide. We are dedicated to delivering outstanding,
personalized care that enhances health, reduces costs, and empowers
individuals to lead fulfilling lives. With a strong foundation in
clinical assessment and care management, our extensive network of
clinicians transcends traditional barriers by meeting members in
their own environments. We aim to assist older adults and other
at-risk populations in achieving better care quality, improved
health outcomes, and the identification of chronic conditions that
might otherwise remain undetected. Joining our team means utilizing
your skills and compassion to make a tangible difference in the
health and well-being of others. We offer competitive compensation
and flexible scheduling, allowing you to contribute positively to
the community! Responsibilities About the Role During visits that
may last up to one hour, our providers assess and observe a
member’s current health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) enhances care quality
and enables us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, which include reviewing
medical history, diagnoses, treatment plans, health education,
physician referrals, case management referrals, follow-ups, and
thorough documentation in line with our guidelines and protocols.
Administering and ordering necessary point-of-care and lab-based
tests. Collaborating with physicians, case managers, social
workers, family members, key caregivers, and other medical
personnel as needed. Working alongside Primary Care Physicians
(PCPs) to educate patients and ensure appropriate follow-up.
Providing services at the member’s residence, whether in their
homes or in Skilled Nursing Facilities. Benefits of Working with Us
Flexibility – Schedule visits according to YOUR availability.
Assessments are conducted from Sunday to Saturday, between 8 am and
7 pm. You can choose to work during the day, evening, or weekend

based on the number of assessments completed. You can increase your
income by being more available and traveling to see members outside
your home area. Those averaging 20 to 100 visits monthly can earn
between $1,800 and $11,500 , depending on the per assessment rate
of $90 to $115 , which varies by state. Benefits – Includes mileage
reimbursement, a 401(k) plan, employee referral bonuses, and
limited voluntary benefits. Education and Support – We prioritize
your development! Along with comprehensive onboarding training, you
will be paired with a preceptor, receive personalized feedback, and
gain free access to continuing education opportunities. Additional

obtaining additional state licensure and credentialing in
neighboring states or others you wish to visit while conducting
health assessments. Full-time Opportunities – Our PRN providers
