Home is seeking a Clinical Team Leader who brings heart, expertise,
and a steady presence to our dedicated clinical staff and the
patients and families we serve. This role is central to ensuring
exceptional continuity of care, supporting our nursing and hospice
aide teams, and fostering a collaborative, mission?driven
environment. You’ll lead with empathy, clarity, and
confidence—helping your team deliver the highest standard of
hospice care. The schedule for this position is 40 hours per week-
Monday-Friday 3:00pm-11:00pm; occasional weekends and holidays as
scheduled. What You’ll Do Lead & Support Your Team Coordinate
interviewing, hiring, and onboarding of new clinical staff Support
training, coaching, and ongoing development for nurses and hospice
aides Create a positive, collaborative environment where staff feel
supported and valued Manage scheduling, performance evaluations,
and personnel actions Provide day?to?day supervision for RNs, LPNs,
and hospice aides on your assigned shift Ensure Exceptional Patient
Care Direct, prioritize, and coordinate patient care delivery for
Hospice Home patients Assign admissions appropriately and ensure
smooth transitions Partner with patients, families, physicians, and
interdisciplinary team members to resolve care needs Oversee
documentation accuracy, timeliness, and regulatory compliance
Conduct weekly chart audits and support continuous quality
improvement Strengthen Clinical Operations Oversee medication room
operations, including narcotic compliance Support medication
delivery processes from the pharmacy Review and update clinical
policies, procedures, and workflows Identify competency and
training needs in partnership with PI and Education teams Ensure
adherence to ACHC Standards, Medicare CoPs, and all state/federal
regulations Additional Responsibilities Serve as Management Back Up
(MBU) as assigned Cover at least one holiday annually Participate
in workshops and seminars to support professional growth Manage
time, people, and technology with professionalism and efficiency
What You Bring Current RN license in North Carolina 5 years of
recent clinical experience in hospice, home health, or related
field 2 years of supervisory experience preferred Strong
understanding of hospice clinical interventions and protocols
Knowledge of Medicare/Medicaid regulations, reimbursement, and
accreditation standards Excellent communication, leadership, and
team?building skills Competence with Windows-based programs and
willingness to learn Allscripts Current CPR certification Valid NC
driver’s license and reliable transportation Ability to lift up to
