Job Description:
MindPath Care Centers, a part of Community Psychiatry, is
seeking a dedicated Outpatient Hybrid Psychiatric Mental Health
Nurse Practitioner (PMHNP) to join our multidisciplinary team of
mental health professionals. In this role, you will provide
comprehensive psychiatric care through both in-person and
telehealth platforms, ensuring flexibility and accessibility for
our patients. You will work collaboratively with therapists and
other medical professionals to deliver integrated, patient-centered
care designed to meet the unique needs of those we serve. Our focus
is on innovative approaches to mental health treatment, grounded in
empathy and scientific understanding. Perform psychiatric
evaluations and medication management for a diverse outpatient
population. Develop treatment plans based on scientific evidence
and personalized patient needs. Engage in ongoing collaboration
with therapists, psychologists, and other healthcare professionals
to ensure comprehensive care. Participate in clinical review and
team meetings to discuss patient treatment and progress. Stay
current with advancements in psychiatry and mental health
treatment, incorporating new techniques as appropriate. Provide
crisis intervention as required. Maintain electronic health records
with accuracy and integrity. Strong diagnostic capabilities
Proficient in the use of electronic health record systems Excellent
communication and interpersonal skills Ability to work effectively
in both independent and team environments Adaptable to both
face-to-face and digital communication with patients Medical

Active licensure as a Psychiatric-Mental Health Nurse Practitioner
(PMHNP) in the state of practice Board certification as a PMHNP CPR
certified
