with a new program!RHA is seeking a fully licensed clinician LCAS,
LPC, LCSW to lead the development of a new service in Alamance
County Critical Time Intervention. The licensed Team Lead will work
with a team of three Certified Peer Support Specialists/Qualified
Professionals to serve individuals returning the community from
in-patient and institutional settings.Critical Time Intervention
(CTI) is an intensive 9 month case management model designed to
assist adults age 18 years and older with mental illness who are
going through critical transitions and who have functional
impairments which preclude them from managing their transitional
needs adequately. CTI promotes a focus on recovery, psychiatric
rehabilitation and bridges the gap between institutional living and
community services. CTI differs from traditional case management
because it is time limited, focused and follows a three phased
approach.Individuals receiving CTI are navigating a critical
transition and are not connected to other community based services
currently meeting their clinical needs. Pay Range: $55,000.00 to
$65,000.00 Critical transitions include the following: Discharge
from psychiatric inpatient settings Release from correctional
settings Transition out of foster care settings into adult services
Transition from homelessness into housing Transition from highly
structured residential settings, such as adult care homes, into
independent living Job Requirements: Graduate degree in Counseling
or a related field Valid licensure to provide mental/behavioral
healthcare in the state of North Carolina Licenses include LCAS,
LPC, and LCSW Minimum 2 years experience working with individuals
with mental and/or behavioral health issues that include writing
person-centered plans, conducting effective group therapy and
providing mental health diagnoses General working knowledge of the
human services delivery system in North Carolina Valid driver's
license, auto insurance and reliable transportation Ability to pass
a drug screen and background check Ability to lift between 20 and
50 pounds and meet the physical requirements of performing CPR and
therapeutic holds Employee supervisory experience preferred
Experience with Electronic Medical Records (EMR) preferred
