In this role, you will be vital in providing mental health services and support, facilitating patient assessments, developing treatment plans, and promoting patient mental health and wellbeing through compassionate care. This position offers the opportunity to work in dynamic settings while ensuring high standards of culturally competent care for a diverse clientele. Conduct comprehensive patient assessments to identify mental health conditions and needs Develop and implement individualized treatment plans based on patient needs and evidence-based practices Facilitate group and individual therapy sessions to assist patients in overcoming emotional, cognitive, and behavioral issues Collaborate with healthcare team members to ensure holistic care and treatment continuity Maintain detailed records of patient progress and services provided, complying with all legal and professional standards Advocate for patient access to community resources and services Educate families and caregivers on patient mental health and wellness strategies Expertise in various forms of therapy, including cognitive behavioral therapy and motivational interviewing Strong interpersonal and communication skills Ability to handle sensitive information confidentially Proven competency in treatment planning and documentation Capacity to work independently and as part of a multidisciplinary team 401k Current and valid LCSW license Certification in clinical supervision preferred
